“Henryetta After-School Program”

Parents/Guardians:

Please read the following information packet about the HASP program. If you choose for your child to attend, please fill out and sign the Financial/Policy Agreement and Registration form, and return that sheet only as soon as possible.

PROGRAM:  The HASP (Henryetta After-School Program) is sponsored by Henryetta Public Schools. The program is for children in grades Pre-K thru eighth grades, and will offer a place for the students to be able to develop physical, mental, and social developmental skills. The Parents are encouraged to contact the HASP office with any questions and/or concerns.        

GOAL:  The goal of the HASP (Henryetta After-School Program) is to provide a safe, healthy, and secure environment for Henryetta students where they can participate in enrichment activities outside of regular school hours.   

QUALIFICATIONS:  The HASP (Henryetta After-School Program) is available to students enrolled full-time in the Henryetta Public Schools in Grades Pre-K thru eighth. If the student has not attended school on any given day, he/she will NOT be permitted to participate in HASP on that day.

COMMUNICATION: Information is communicated by using newsletters sent home with students, emails, posting at the site and HASP office, Henryetta Public Schools website www.henryetta.k12.ok.us , and local newspapers.

DAYS IN SESSION:  The program will be Monday through Friday. The program is only open when school is in session. Please see the current school calendar for date’s school is closed. When Henryetta Public Schools classes are canceled for any reason, the HASP afterschool program will be closed.

HOURS IN SESSIONThe HASP (Henryetta After-School Program) begins promptly at the end of the school day. Parents and guardians are expected to pick up their students no later than 6:00 p.m. each day.

ENROLLMENT:  Enrollment in the program is completed when the parent or guardian has:
                                *Completed the enrollment contract forms
                                *Paid the non-refundable registration fee ($20 per family)
                                *Read and agreed to all policies stated in the handbook

NOTE: The HASP office must be informed of any changes in emergency information or Payment information.

TUITION: A copy of a financial contract will be given upon registration in the program.
                    *Monthly payments are due the first day of the month.
                    *No credit will be given for absences or school closings.
                    *Fees are subject to change.
                    *Notice will be given to parents in the event of a fee increase.
                    *Payments may be made by check, money order or cash.

                    *Tuition paid beginning September 1st, and paid monthly for the remainder
                      of the school year will result in “NO” tuition required for the month of May.

                    *Tuition is Tax Deductable. 

Tuition must be paid in full on or before the 8th of each month, or an invoice will be mailed or emailed on the 9th day of that month with a $25.00 late payment fee. Tuition (plus applicable fees) must be paid in full by 5:00 pm on or before the 15th of each month. A DISMISSAL NOTICE will be sent on the 16th of the month and your child will be withdrawn from HASP on that date.

How to Make Tuition Payments:
       *Mail (1801 Troy Aikman DR. Henryetta, OK 74437)
       *Board of Education, Henryetta Public Schools, from 8:00 am to 3:30 pm, Monday thru         
         Friday.

LATE FEES: A $25.00 late fee is applied on the ninth (9th) program day of each month.
*All returned forms of payment will be assessed a $15.00 return fee.
*The amount of the returned form of payment, plus the $15.00 fee, must be made in cash, money order, or certified check within two (2) days after notification from the HASP office.
*Failure to do so may result in the child(ren) being removed from the HASP program.
*After two (2) returned checks, payment is only accepted in cash or money order.
*Tuition (plus any applicable fees) must be paid in full by 5:00 pm on or before the 15th of each month. No child will be allowed to attend HASP beginning on the 16th of the month.
*A late charge of $1.00 per minute per child will be assessed beginning at 6:01.
*If a student is dismissed from the program due to non payment, reoccurring payment issues or behavior issues, student cannot be re-enrolled for one year from date of dismissal.

SNACKEach afternoon a snack will be provided for the students at no charge.

RELEASE OF STUDENTS:  You must provide HASP with information as to how your student will be released from the program each day. Parents must come in the building and sign out the child. Individuals authorized to sign out students must be listed on the enrollment form. It is best to list names of several different individuals in case you are delayed. If the person signing out the child is not known personally by the Site Coordinator or Director, photo identification will be required. Elementary students are not allowed to either walk or ride bicycles to or from the Elementary School. Therefore, an authorized adult must sign them out daily. Children must be picked up by 6:00 pm. NOTE: A late charge of $1.00 per minute per child will be assessed beginning at 6:01 pm. Continued late pick-up may result in termination of the child in the HASP program.

 (Early Release) If you encounter an emergency situation that will prevent you from picking up your child on time, you must make arrangements for someone else to pick up your child and notify the Site Coordinator or Director.

NOTICE: The HASP Program must be notified if there is a court order prohibiting certain persons from having contact with your child.

EMERGENCY PROCEDURESShould an emergency situation arise (such as ice or snow, loss of water, etc.) immediately before or during HASP time, the staff will notify parents of the existing situation. In such circumstances, parents will be required to make arrangements for immediate pick-up of their student.

MEDICAL AND EMERGENCY INFORMATIONWe have asked you to provide this information in order to better serve and protect your child. We ask you to keep your child home if they show any signs of a contagious disease in the past 24 hours, if a child becomes ill while at the program the parent will be notified to pick up the child within one (1) hour.  It is the parent or guardian’s responsibility to notify the HASP personnel if there are any changes in the parent contact and/or emergency information.

ILLNESS/ACCIDENTS:  Should your student become ill or injured during the program, he/she will be brought to the office where we will try to determine the nature and severity of the illness. If the student cannot, in our judgment, remain at the program, we will contact the parents or the emergency phone number. It will be necessary for you to make arrangements to pick-up your student immediately.  

BEHAVIOR/DISCIPLINE POLICYStudents are under the supervision of qualified personnel who are familiar with the rules set by the Henryetta Public Schools. The HASP rules are the same as for the regular school day. Staff will act as positive role models. Rules will be consistently implemented. If behavior problems occur the following steps will be used,
*Discuss the concern with the child.
*Discuss the unacceptable behavior with the parent.
*Write an incident report for the parents to sign.
*Suspend services for one (1) to three (3) days if the inappropriate behavior continues.
*Drop the child from the program if behavior continues after the child returns from the suspension. No refunds.

CAUSE FOR IMMEDIATE DISMISSAL:

HASP will not continue services for a child:
*When the child’s behavior endangers him/herself or others.
*When intentional destruction of property occurs.
*When behavior is consistently disruptive to the entire program affecting all students.
*When the child leaves the site/program without permission.
*For, non-payment, late payments, pick-up fees or continuous late pick-up.
*If a parent or guardian physically or verbally abuses/threatens another child.
*If a parent continually fails to follow policies and procedures of the program.

The program reserves the right to dismiss any child if the staff determines that the program cannot meet the needs of the child. In extreme cases, the parent may be requested to pick the child up during program time.

Failure to comply with the terms of the parent contract may result in termination from the program.
