***SCHOOL WILL RESUME IN PERSON ON OCTOBER 12TH.  MASKS WILL BE REQUIRED OF ALL STUDENTS/FACULTY AGE 10 AND OVER.

 

(HMS Students entering 7th Grade: Student entering the 7th grade this year must have their TDAP booster, and documentation must be provided to the school before they may attend.)

 

IMPACT AID SURVEY FORM (CLICK HERE)

Henryetta School Parents/Guardians:

Please fill out the Impact Aid Survey Form (one per household). It is very IMPORTANT we have everyone fill this out because it helps FUNDING for our school. Forms will also be sent home in the meal deliveries and are also available to be picked up from the Board Office. Please have all forms returned/fill out no later than Oct. 1.

 

BUS TRANSPORTATION FORM

As Henryetta Public School looks at making the decision to return for the 2nd 9 Weeks, we need to gather information about bus transportation needs if students were to attend two days per week (for example:  M and W).  IF YOUR STUDENT WILL RIDE A BUS, please fill out one Bus Transportation form for each student who will be riding.

ACT Stakeholder Survey

In compliance with Section 1111(b)(2)(H) of ESSA, Henryetta Public Schools intends to choose the ACT as their College and Career Readiness Assessment selection and would like to provide community stakeholders with an opportunity to provide meaningful input regarding our district's intent to request permission to administer the ACT in lieu of the statewide assessment. Muscogee Creek Nation Virtual Learning Support Program

This program is to provide a one-time educational support payment of $850.00 to individual Muscogee (Creek) Nation (MCN) students whose learning opportunities have been negatively impacted as a result of the Coronavirus Pandemic. Eligibility: MCN citizen, and must be an enrolled student (PreK-12, Vocational-Technical, Post-Secondary, or Graduate school) for the Fall 2020 school term. Must be able to verify school enrollment. Students under the age of 18 must have a parent/guardian to complete the application for them to receive the funding.
